Health Insurance Plans


Phantom Eagle offers a health care plan through Blue Cross Blue Shield to help protect you from high medical costs and the financial burden associated with family health care needs.  


Our health plan offers comprehensive care—including coverage for hospitalization, surgery, routine and preventive care office visits, prescription drugs, mental health/substance abuse care, chiropractic services, and many other benefits and services - all with no co-pay.




Phantom Eagle's Vision Plan is an optional benefit that provides comprehensive coverage for services such as eye exams, frames, lenses, and contact lenses following a defined schedule of benefits. You are also eligible for discounts on laser vision corrective surgery. The plan is administered by Vision Services Plan (VSP) which offers a nationwide network of more than 22,000 providers. When you receive vision care services from a VSP network provider, you are covered at the maximum benefit amount. You also have the choice of using an out-of-network provider but will receive a reduced reimbursement.




Phantom Eagle offers a subsidized dental plan that covers the reasonable and customary charges for a wide range of dental treatments at either 100% (preventive) with no deductible or 80% (basic and major). Members can also take advantage of discounted rates when they use dentists who are part of the nationwide network of Delta Dental, our dental administrator. The plan pays up to $1,500 per member for orthodontic services.


Life Insurance


The Phantom Eagle Life Insurance Program offered through Dearborn National helps provide financial security and protection for you and your family.  The company-paid Basic Coverage every member receives includes coverage for spouses and dependents.  Voluntary coverage is also offered at a very affordable rate.



Accidental Death and Dismemberment (AD&D)


Accidental Death and Dismemberment (AD&D) insurance is also included in the Basic Coverage Phantom Eagle provides automatically at no cost to the employee. As with our Life Insurance, voluntary coverage may be purchased at affordable rates.



Short-Term Disability


Phantom Eagle's Short-Term Disability insurance is a company-paid benefit that accommodates a documented disability. It can be used for an acute illness or injury or for a chronic condition that results in intermittent absences. 60% of member's weekly earnings are payable beginning on the 8th day of injury or sickness, and continues for up to 13 weeks.  


Long-Term Disability

Also a company-paid benefit, Long-Term Disability insurance provides income replacement for a non-work related illness or injury that continues past the Short-Term Disability period, which is 90 days.  The plan provides for a benefit equal to 60% of base monthly salary. 


401K Savings Plan


One of Phantom Eagle's most significant benefits is the Retirement Program, offered through ADP Retirement Services, which offers employees the option of pre-tax contributions and a choice of numerous investment options ranging from conservative to aggressive. Key features of the Program include immediate 100% vesting (ownership) of all contributions, the flexibility to change and reallocate contributions at any time, and generous company matching contributions.



Phantom Eagle recognizes ten official holidays each year: New Year's Day, Martin Luther King, Jr. Day, Washington's Birthday, Memorial Day, Independence Day, Columbus Day, Labor Day, Veterans Day, Thanksgiving Day, and Christmas Day.
